Requirements
Annual deadlines for postmarking proposals:
March 15th and September 15th.

Prior to submitting a request (at least two weeks before the deadline), please call the Foundation Director to discuss your request.

Board meetings are held twice each year in May and November. Award letters will be mailed out within three weeks of the Board meetings.

Submit one unbound copy of your proposal and limit the length to 6 pages exclusive of attachments. If you wish to submit additional materials, please be selective!

Briefly address the following concerns in your proposal narrative. The Foundation also accepts the Minnesota Common Grant Form. Requests will be considered only when all materials have been postmarked or received by the dates indicated above.

ALL PROPOSALS:
All proposals submitted to HRK must include:
  1. A Cover Sheet (please use the form included here or the Minnesota Common Grant Form).
  2. A copy of your IRS Section 501(c)(3) Tax Determination Letter - or similar information from your fiscal agent.
  3. A copy of your most recent audited financial statements or IRS Form 990.
  4. An organizational or project Budget (please use the form included here or you may include an existing single-page budget).
CURRENT GRANTEES:
If you received general operating support last year, and are asking for renewed support this year, please also submit:

  1. A two-page letter indicating any changes or concerns from the past year and your plans for the coming year.
NEW APPLICANTS:
If you are a new applicant to HRK Foundation, we would also like to receive:
  1. Organizational Information:
    • A brief summary of your organization's highlights including: history, mission and goals, current programs and activities, service statistics, board members and key staff, and your relationship with other organizations working to meet the same needs or providing similar services.
  1. Purpose of the Grant:
    • A brief summary highlighting the situation, specific activities, goals, and the timeframe involved.
  1. Evaluation:
    • A brief summary of how you define success and who will be involved in evaluating the potential outcomes.
